Guest Liability Waiver
All non-members that enter upon the Suffolk Archers Archery Park, whether to shoot or observe, must complete and sign this waiver of liability. It can be downloaded and printed here, and must be deposited in the box on the 10-30 yard range, in the appropriate envelope.
Blank forms are also available at the Archery Park in this box.
NOTE THAT WAIVERS FOR MINORS MUST BE SIGNED
ONLY BY A PARENT OR A LEGAL GUARDIAN.
Grandparents or other relatives cannot legally sign this waiver for grandchildren or other minors. Have parents sign this waiver for their children before bringing them to our Archery Park.
